- Enrollment Application / Child Information Form
- Emergency Contact & Authorized Pick-Up List
- Immunization Record (or exemption affidavit)
- Health Statement (Form 2936) –Signed by child’s doctor
- Food Allergy / Dietary Needs Form
- Allergy or Medical Action Plan (for EpiPen, asthma, etc.)
- Infant Feeding Plan (for children under 12 months)
- Discipline & Guidance Policy Acknowledgment
- Parent Handbook Acknowledgment
- Photo / Video Consent Form
- Tuition & Financial Policy Agreement
- Late Pickup Policy Agreement
- Field Trip / Water Permission Form (if applicable)
